When offering orthodontic treatment, the choice between labial and lingual appliances is often determined by many factors. Both will likely have their strengths and weaknesses in achieving the desired result for a certain case. It’s therefore important to consider these factors thoroughly and walk through them with patients in order for them to make an informed decision.

However, due to significant developments in lingual appliances, it is undeniable that these solutions are now able to offer a number of benefits that make them a highly effective, and often preferable choice for a diverse range of patients.

A highly experienced professional with lingual appliances, Dr Ektor Grammatopoulos is hosting a seminar exploring lingual appliances at 3M Oral Care’s Excellere 2019. Titled “Opening and closing spaces with Incognito™: practical advice”, Dr Grammatopoulos’ session will identify the different patients who will benefit most from lingual appliances and how to harness the power of these systems to close spaces and produce predictable, controlled tooth movement during treatment.

Suitable for a wide array of patients

Dr Grammatopoulos says: “It is well known that lingual appliances are particularly useful in patients who wish to undergo orthodontic treatment with absolute discretion due to work or personal circumstances. My lecture will start with the clinical illustration of less common circumstances where lingual appliances are invaluable.

“In a modern era where appearance is key, more and more patients are looking for orthodontic solutions that can deliver the expected results without compromised aesthetics during treatment. Many people now work in front-facing industries where their smile is an important asset, or they may have various other reasons for preferring invisible treatment.

“There are also certain less common circumstances where lingual appliances become invaluable. For example, patients with vertical maxillary excess or those with a higher percentage of visible gingiva when smiling are more likely to choose lingual appliances, as they don’t want to draw attention to their mouths. Furthermore, those who play brass instruments who do not want to risk alterations in their performances are more likely to opt for a lingual solution so that they can continue their music without fear. Adult patients with scars on their lips such as those who were born with a cleft palate are also expected to choose lingual appliances – perhaps as they want to avoid unnecessary attention to their oral region.”

Necessary knowledge

In many ways, lingual appliances can feel like a more daunting option as professionals are often unsure about what using these appliances entails. In light of this, it’s a good idea for clinicians to learn more about the devices and how to use them in certain cases in order to get the best results.

One of the main focuses of Dr Grammatopoulos’ session at Excellere will be closing spaces that are the result of extraction/tooth loss or congenitally missing teeth (hypodontia).

Dr Grammatopoulos says: “Opening and closing spaces following the loss of teeth or in hypodontia cases can present orthodontists with challenges in both lingual and labial techniques. My lecture will provide practical advice for efficient and controlled tooth movement.

“Although opening and closing spaces can be some of the most challenging movements in lingual orthodontics, optimum control of teeth prevents unfavourable effects and can expedite the progress.”
